    DID YOU KNOW?

    This is the last week the Honors Student Council is accepting donations for its Book and Media Sale (BAMS), which is next week. Drop off books, CDs, records, games and DVDs in one of the BAMS boxes around campus. All of the proceeds from the event go directly to Camp Quality Kentuckiana, which gives children living with cancer the chance to be a kid again at summer camp.

    1.) You can now update your W-4 information electronically

    Payroll Services is pleased to announce the online W-4 Tax Information form.  You can now update, electronically sign and submit your W-4 Tax Information online through our self-service portal.  Simply login to ULink, go to the Faculty & Staff tab, navigate to Personal Information and click “W-4 Tax Information.” Here you can change your allowances, update your marital status, edit your number of dependents, withhold additional amounts from your paycheck, etc. Additional instructions are available on the Payroll Services website.

    2.) New IRS Tax Form 1095-C to be mailed

    The Affordable Care Act (ACA) requires employers to provide the IRS Form 1095-C to benefit eligible employees. This form will be mailed to your home this week and provides proof of health insurance coverage offered to eligible employees and the dependents covered under our group health plan during 2015. You do NOT need this form to file your personal tax return.

    5.) Two student info sessions about university tuition scheduled

    April 1, noon, HSC, Medical School B-Building, Room B102 (lunch will be provided); April 11, noon, SAC Floyd Theater, Belknap (refreshments will be provided)

    Two information sessions regarding University tuition have been scheduled for the campus community.  All students are invited to join representatives from the Finance Office, and Student Affairs in a discussion about tuition and how tuition is set at the University of Louisville. Questions and feedback are welcomed.

    6.) Recruiting students for the Welcome Week 2016 Crew

    The Office of First Year Initiatives is hiring a small group of students to join the FYI crew from Aug. 8-21. Applicants must be able to work during the timeframes listed on the web form and cannot be a part of a group that already has Welcome Week work commitments. Priority will be given to students who apply by April 11 and FYI will notify candidates promptly.

    8.) Chinese spoken drama showcase includes skits and music

    UofL students studying Chinese will present a series of Chinese skits and music on Friday. Refreshments will be served at 12:30 p.m. This is an opportunity for our students to share their unique experience of learning Chinese and Chinese culture with the university community.

    11.) Visiting Artist Lecture to be presented by Nick DeFord

    Nick Deford’s work explores the visual culture of cartography, occult imagery, geographical souvenirs, and other structures of information that is altered to examine the relationship of identity, space and place. Incorporating found objects, maps, books, paper and embroidery, DeFord’s work investigates the physicality of stitching and manipulation of these materials as he examines locational devices that locate both physical place and personal identity.

    16.) Adult Siblings of Individuals with Childhood Cancer needed for participation in research study

    We are looking for people age 18+ who grew up in the same household with a sibling who was diagnosed with cancer prior to his/her 19th birthday to complete an online survey. The information you provide may assist counselors in identifying special issues when working with adult siblings of individuals with cancer and lead to improved care.     Medicine Grand Rounds features Laurel J. Lyckholm, MD, FACP, Professor of Medicine in the Division of Hematology/Oncology/BMT at the University of Iowa Carver College of Medicine. Dr. Lyckholm will present “Truth or Consequences: Prognostication in Cancer,” focusing on reasons the provision of accurate prognosis informs ethical cancer care, primary factors that determine prognosis, differentiating between Karnofsky and ECOG Performance Status, and ways prognosis may be provided to patients.
